Tree nuts, including almond (prunus dulcis) are a source of food allergens often associated with life-threatening allergic reactions in susceptible individuals. Although the proteins in almonds have been biochemically characterized, relatively little has been reported regarding the identity of the allergens involved in almond sensitivity. The present study was undertaken to identify the allergens of the almond by cDNA library approach.
cDNA library of almond seeds was constructed in Uni-Zap XR lamda vector and expressed in E. coli XL-1 blue. Plaques were immunoscreened with pooled sera of allergic patients. The cDNA clone reacting significantly with specific IgE antibodies was selected and subcloned and subsequently expressed in E. coli. The amino acids deducted from PCR product of clone showed homology to 60s acidic ribosomal protein of almond. The expressed protein was 11,450 Dalton without leader sequence. Immunoreactivity of the recombinant 60s ribosomal protein (r60sRP) was evaluated with dot blot analysis using pooled and individual sera of allergic patients.
The data showed that r60sRP and almond extract (as positive control) possess the ability to bind the IgE antibodies. The results showed that expressed protein is an almond allergen.
Whether this r60sRP represents a major allergen of almond needs to be further studied which requires a large number of sera from the almond atopic patients and also need to determine the IgE-reactive frequencies of each individual allergen.
Universal vaccination of neonates and children against diphtheria, tetanus and pertussis has had a tremendous impact on the control of these infectious diseases worldwide. Immunization by the triple diphtheria, tetanus and whole cell pertussis vaccine (DTwP) has been applied in Iran for almost 50 years. Periodic assessment of immunogenicity of this vaccine is an important aspect of successful mass vaccination programs. The present study was performed to assess the antibody response against tetanus, diphtheria and pertussis in a group of Iranian infants vaccinated with a local DTwP vaccine.
In this prospective study, 330 infants received primary vaccination at 2, 4 and 6 months of age with DTwP vaccine manufactured by Razi Institute of Iran. Blood samples were taken 2-4 weeks after the third dose to assess seroprotection and geometric mean titers (GMT) of specific antibodies.
Among the 283 infants who completed the vaccination course, 98.2% and 100% developed antibodies against diphtheria and tetanus, respectively. The GMT of antibodies to tetanus, diphtheria and pertussis, were 2.09 IU/ml, 2.08 IU/ml and 8.73 EU/ml, respectively.
Comparison of the results obtained from this study with those from previous studies performed in other countries revealed a similar GMT and protection rates for diphtheria and tetanus components. In the absence of well-established serological criteria, judgment about protection rate against pertussis has not been possible. A prospective vaccination study using the local DTwP vaccine in parallel to a WHO approved standard vaccine, could enable assessment of immunogenicity of the pertussis component.
Adult periodontitis is a complex multifactorial disease whose etiology is not well defined. The pro-inflammatory and bone resorption properties of Interleukine-1α (IL-1α) strongly suggest a role for this cytokine in the pathogenesis of periodontal disease.
Eighty Iranian adult patients with periodontitis and 80 Iranian controls were investigated in this study.
In this study we report that the frequency of IL-1α genotypes including allele 2 of the IL-1α(-889) restriction fragment length bi-allele polymorphism were significantly increased in patients with advanced aggressive adult periodontitis compared to those with early and moderate disease. Furthermore, allele 2 was associated with increased production of IL-1α by activated peripheral blood polymorphonuclear cells of patients with advanced disease, although this increase failed to reach statistical significance. Finally, the data obtained revealed significant linkage disequilibrium between allele 2of the IL-1α (-889) polymorphism and allele 2 of the bi-allelic IL-1β (+3953) polymorphism in both patients and orally normal controls.
These findings provide new insight into the possible rate of IL-1α and β genes polymorphism in the susceptibility to adult periodontitis.
Acute lymphoblastic leukemia (ALL) is a malignant disorder of lymphoid precursor cells, which could be classified according to morphological and cytochemical methods as well as immunophenotyping.
Twenty patients with ALL, who had been referred to the Children's Medical Center Hospital, during the year 2007, were enrolled in this study in order to evaluate the morphologic and immunophenotypic profile of these patients.
Cytologic analysis of blood and bone marrow samples revealed that the frequency of ALL-L1 was 70%, followed by ALL-L2 and ALL-L3. The onset age of the patients with ALL-L1 was significantly lower than the patients with L2/L3. Severe anemia was significantly detected more in L1 group. Flow cytometic study of bone marrow showed that 10 cases had Pre-B1 ALL and 7 cases had Pre-B2 ALL, while three cases had Pro-B ALL. Comparisons of the characteristics and clinical manifestations among these groups did not show any appreciable difference. There were an increase percentage of CD20+ cells and a decrease CD10+ cells in pre-B2 group in comparison with pre-B1 group. Fifteen patients were in standard risk and five were in high risk. Although standard risk patients were more common in the group of pre-B1, this was not significant.
Our results confirm the previous reports indicating heterogeneity of ALL. Immunophenotyping is not the only diagnostic test of importance, while morphological assessment still can be used in the diagnosis and classification of the disease.
Bakers are frequently exposed to various irritant chemicals during work which can induce respiratory problems. In this study, pulmonary function tests and self-reported respiratory and allergic symptoms in bakers were compared with matched control subjects.
The frequency of respiratory and allergic symptoms was evaluated in a sample of 58 Iranian bakers and 58 control subjects using a questionnaire. Pulmonary function tests (PFT) were also measured in all participants.
All respiratory symptoms were significantly higher in bakers than control croup (P<0.05 to p<0.005). All allergic symptoms in bakers were also significantly greater than control group (P<0.05 to p<0.005). In addition, all respiratory (except sputum) and allergic (except urticaria) symptoms were significantly higher in bakers during work compared to rest period (p<0.05 to p<0.01) during work. Most PFT values except MEF25 were also significantly lower in bakers than control subjects (p<0.05 to p<0.001).
These results showed that bakers have a higher frequency of work related respiratory symptoms and to a lesser extend allergic symptoms particularly during the work period. PFT values were also significantly reduced among bakers.
Churg Strauss Syndrome (CSS) is a form of primary vasculitis that is characterized by severe eosinophilia and often granulomatous inflammation as well as history of asthma or allergy. Previously, the association between cysteinyl leukotrien receptor antagonists (LTRAs), corticosteroid withdrawal or a sudden change in its used method and CSS had been established.
We report three cases that have been referred because of dyspnea, wheezing and cough with a history of allergic rhinitis and nasal polypectomy. After polypectomy, disseminated skin purpuric rashes appeared on their forelegs, abdomen and all of them had experienced neuropathic signs in their extremities. Clinical findings, marked eosinophilia in blood and skin biopsies finally led to the diagnosis of CSS. The patients have been free of symptoms after receiving prednisolone; routine examinations and blood tests have rended regular results.
Here, we report a probable occurrence of an association between nasal polypectomy and CSS on the basis of our findings. Further, extended researches are required to establish this correlation.
We describe the development of enterovirus meningoencephalitis associated with increased adenosine deaminase in cerebrospinal fluid of a 12-year-old boy, a known case of hypogamaglobulinemia despite monthly replacement of IVIg.
The patient was referred to our center with fever, headache and vomiting for 10 days. CSF analysis was compatible with aseptic meningoencephalitis but high CSF protein (>200mg/dl) and high level of adenosine deaminase in CSF (30IU/L) were against the diagnosis of simple viral meningoencephalitis. Nested PCR of CSF for entrovirus was positive. Treatment with daily high-dose IVIg was commenced, with significant clinical improvement.
For patients with increased ADA and lymphocytic pleocytosis in CSF, differential diagnoses should include enteroviral meningitis. Antibodies, although crucial, cannot on their own prevent enteroviral infection in some hypogamaglbulinemic patients.
This study was conducted to evaluate the role of swimming on mechanic of lung in healthy individual and patients with asthma. A total 76 girls who took part in the course of regular swimming session three day per week for eight weeks enrolled in this study. All of them completed ISAAC written questionnaire and individual who was suspected of asthma or other atopic diseases was referred to allergist for more evaluation. Peak expiratory flow rate was recorded for participants at beginning, one hour after swimming and two months later. According to ISAAC questionnaire 35.4% had asthma or other atopic diseases. Increase in PEFR more than 20% of personal best was seen in 21.9% after one hour swimming and in 27.6% after two months. Increase in PEFR was significant in healthy individual and asthmatic patients and obese but was not significant in patients with allergic rhinitis or eczema. This study suggests swimming in indoor pool is useful for patients with asthma in spite of potential toxic role of chlorine in exacerbation of asthma symptoms and lung mechanics.
