Allergic asthma considered is a type I hypersensitivity disorder with increasing global prevalence and limited curative options. Mesenchymal stem cells (MSCs) and their exosomes have emerged as promising adjuncts in the control of allergic reactions. MSCs can regulate and shift the inflammatory responses and promote tissue remodeling in the airway tissues. Furthemore, exosomes derived from MSCs contribute to these effects by mediating intercellular communication and delivering bioactive molecules that modulate immune responses. This review discusses current evidence on the immunomodulatory potential and therapeutic benefits of MSCs and their exosomes in the context of allergen-specific immunotherapy (AIT) to enhance efficacy, induce long-lasting tolerance, and address current therapeutic limitations in allergic asthma.
